What is IRMAA? Higher Medicare premiums for higher incomes¿Qué es IRMAA?
IRMAA (Income-Related Monthly Adjustment Amount) is an extra amount added to your Medicare Part B and Part D premiums if your income is above a set limit. In 2026 it applies if your modified adjusted gross income (MAGI) is above $109,000 (individual return) or $218,000 (joint return). Social Security uses your tax return from 2 years earlier: 2026 amounts are generally based on your 2024 return. If your income dropped because of a life-changing event like retirement, you can ask for a new decision with Form SSA-44.
